Procedures
Obtaining a work permit is a two step process.
1. Obtain a “Statement of Request to Employ Minor and Request for Work Permit” form from the College and Career Center. The center is open M-F, 9:00am to 3:30pm. The student completes his/her personal information, asks the employer to complete the employer portion and finally obtains their parent/guardian’s signature. When the “Request” is completed and signed, the student must return the form to the College and Career Center.
